Your first visit will consist of a consultation so that Dr. Ken may meet you and thoroughly understand your health concerns. He will examine your spine and problem areas in order to determine if chiropractic care can help you. In most cases, spinal x-rays will be taken in order to provide a blueprint of your spine and lend additional insight to your condition. Your x-rays will be analyzed and examined in front of you so that you may also benefit from the information they provide. Finally, you will receive your first adjustment (treatment) assuming that we can help you.
We have included the new patient paperwork so that you can download and print it ahead of time. This will not only allow your first visit to move more efficiently, but will also give you the time to document your condition carefully and thoughtfully. Please make sure to download the form most appropriate for your situation. Personal injury forms are for those individuals involved in an accident where litigation is being pursued; Worker’s compensation forms are intended for patient’s that have been injured at work and are pursuing compensation from their employer. All others should use the standard intake form.
